1) Physical Exam Form - Must be signed by your doctor

Physical Examination requirement
- Emmaus Athletics requires all student-athletes on EU intercollegiate sports team to obtain physical examination clearance from a qualified medical physician prior to reporting for their respective preseason.
- Before you are allowed to participate in any team activities [e.g., practices, competitions, conditioning, weight training], you must be medically cleared and have your signed Physical Exam Form, Medical Health History Form, and Health Insurance Card uploaded to your Populi webpage.
Health Insurance
- All student-athletes must have personal health insurance and upload a photo of their Health Insurance Card to Populi. Proof of insurance is also required for all residential students at Emmaus, and is mandatory for all student-athletes prior to participation in any team activity.
- If you do not have a current health insurance coverage plan, you must contact the Residence Life Department to inquire about plan options with Emmaus University. Email the Residence Life office at: reslife@emmaus.edu
